WOMEN, INFANTS AND CHILDREN (WIC) Supplemental foods and nutrition education are provided to women, infants and children through WIC clinics. Visit our monthly clinic calendar for dates and times. EARLY INTERVENTION/CHILD FIND PROGRAM This program identifies young children (birth to age 3) who are developmentally delayed and/or disabled and secures services for those meeting eligibility requirements. PRESCHOOL SPECIAL EDUCATION PROGRAM Special education evaluations and services are provided to children ages 3 to 5 who meet eligibility requirements. CHILDREN WITH SPECIAL HEALTH CARE NEEDS (CSHCN) AND PHYSICALLY HANDICAPPED CHILDRENS PROGRAM (PHCP) Families of uninsured or underinsured children with special health care needs are assisted in obtaining needed services for their child. PHCP provides orthodontic services for income eligible children who demonstrate a medical need. HEALTHY LIVING PARTNERSHIP Breast cancer and colorectal screenings are offered to Hamilton County residents who have no health insurance. For further information please call 800-882-0121. CERTIFIED HOME HEALTH AGENCY The Certified Home Health Agency provides skilled nursing care to patients in the home care setting. Home health aide services, physical, speech and occupational therapy and medical social worker services are also available. Acuity of condition determines the frequency of visits. All insurances are accepted including Medicare, Medicaid and third party payers. A sliding fee scale is available for those without insurance. Referrals are accepted from physicians, hospitals, family and community.
